The Older Persons National Service Framework (NSF) was launched in March 2001, with the aim of ensuring that age discrimination is eradicated and that older people and their carers receive consistently high quality services, wherever they live and whatever their circumstances. Most importantly, the NSF requires closer integration of health and social care, and that these agencies will work together in this pursuit. The NSF also required that a client centred Single Assessment Process (SAP) be established within health and social care communities by April 2004!
The NSF was effectively replaced in 2006 by “A New Ambition for Old Age next steps in implementing the NSF for Older People”.
This project originated in Blackburn with Darwen and the Project Initiation Document (PID) reflected this geographical scope. However the SAP Project Board recommended that the project be extended into East Lancashire which added East Lancashire PCT and Lancashire County Council (east) to the partner agencies. The agencies included in the project are:
