Both organisations welcome this unique opportunity to take a significant step forward to improve the health, care and quality of life for people in Blackburn with Darwen. We have a strong joint vision and will build on our strategic partnership framework, underpinned at service level by a track record in delivering new joint initiatives to meet local need.
Our existing partnerships have delivered a range of new health and social care services that have made a real practical difference to the lives of local people.
Key achievements include:
• Establishing a network of thirteen integrated one-stop Children’s
Centres with outreach services.
• Setting up a fully integrated Independent Living Service
• Delivering healthy living projects
• Creating a new service for older people with mental illness and their
carers.
Building on these partnership strengths, the Primary Care Trust (PCT) and Local Authority (LA) have now agreed robust new arrangements for joint governance and integration of key services, which form the body of the Document below. This new enhanced partnership will include a fully integrated Public Health function, to focus resources on tackling the high levels of deprivation in Blackburn with Darwen.
