Changes to the management of community based NHS Services
From June 1, 2011 the management of the community based NHS services transferred from Blackburn with Darwen care Trust Plus to Lancashire Care NHS Foundation Trust.

In practice the same people will still be providing you with the same community based care you have always received from the Care Trust Plus. The only thing that has changed is that they are now employed by Lancashire Care Foundation Trust.
Under the provisions of the Data Protection Act 1998 Lancashire Care NHS Foundation Trust will become the data controller of your health records in current use or created after June 1, 2011.

Blackburn with Darwen Care Trust Plus will continue to be the data controller of the health records of patients who were discharged from care before this date, and will deal with issues relating to accessing this data.
